Step by Step Process
After completing your Low THC Oil Waiver form, you must sign it in the presence of a licensed notary public. This notarization is a legal requirement for your application to be processed by the Georgia Department of Public Health.

***Do Not Sign in Advance: Wait to sign the waiver until you’re in the presence of the notary.
